The ISB Community Health Clinic will be closed on Monday, January 20 for Martin Luther King Jr. Day. We will be back in office on Tuesday, January 21.

The Islamic Society of Baltimore (ISB) Community Health Center promotes health and wellness by providing quality services, at no charge, to people without access to basic care. We accomplish this through our physicians, nurses, and staff who work with care, compassion, and competence and by respecting the dignity of each individual; serving a diverse community; working collaboratively with volunteers; fostering individual and community partnerships and; responding to the changing health and wellness needs of the community.

Clinic and Education
The ISB Community Health Clinic provides clinical services to uninsured patients during certain hours of the week. Patients must schedule an appointment for medical care by our staff. Our website will strive to provide you information and resources to understand and manage chronic conditions.
However, physical therapy appointments are available by appointment to all, with and without insurance, at a low cost.

Please follow directions below to schedule your appointment with a physician:
No walk-ins - an appointment is required for all office (in-person) or virtual visits.
- Submit the form on the "Request Appointment" tab and a staff member will call you to confirm your appointment.
- Call the Clinic at 410-744-1690 by phone and leave a detailed message.

- Any form of picture ID (i.e. driver's license, passport, etc.),
- COVID vaccination card, and
- List of all of your current medications

Request a Clinic Appointment
Please submit this form to request an appointment with the ISB Community Health Clinic. A submission does not guarantee an appointment. A staff member must call you within 3 business days with a confirmation of the appointment date and time that you have requested.
Please note we only see patients who are uninsured. If you are a new patient, fill out the registration form (under the "New Patient Forms" tab) and bring it with you at your appointment.
If you are 15 minutes past your confirmed appointment time, your appointment will be canceled.
Prescriptions will only be refilled for existing patients.
Physical Therapy is available for everyone regardless of insurance status. Please await confirmation of your appointment date and time from Clinic staff.
Request a Physical Therapy Session
Physical therapy (PT) is charged at $25 per 30-minute session, or $50 per 60-minute session.
Submission of this form does not guarantee an appointment. A staff member must call you within three (3) business days to schedule and confirm an appointment.
Note: If you are 15 minutes past your confirmed appointment time, your appointment will be canceled.

All visits to the ISB Community Health Center are free, but donations are appreciated. Anyone who would like to donate may do so by sending a check or money order to ISB and write Community Health Center in the memo section. Please click here to securely donate online.
